We make every effort to process your insurance pre-determinations and claims in a timely, efficient manner.
We are an in-network provider with Delta Dental only, but are an approved provider with all insurance companies.
Our Insurance Policy is slightly different with Delta Patients so please read carefully for the information that pertains to your situation:
Delta Subscribers only:
If you are eligible for an exam, we will only collect any applicable copay the day of your exam. If you are not eligible, we will collect in full the day of your exam.
After your initial exam visit we will automatically submit your treatment plan to Delta for a predetermination of benefits. Typically Delta will mail both you and us the pre-determination within 3-4 weeks, at which time we will develop a financial agreement for you. Our insurance coordinator will call you to go over this information.
For any insurance other than Delta Dental:
We will collect in full for your initial exam visit but once we receive a predetermination for any future services, we will collect directly from your insurance company for the estimated coverage amount. You will be responsible for the remaining portion of your treatment cost at the time services are provided. You will also be responsible if, for any reason, the insurance company does not cover the originally estimated amount. *
*We make every effort to assure every insurance pre-determination is as solid as possible, including submitting additional information with many claims. If there are any changes to the insurance reimbursement amount once the treatment is completed we will work with your insurance company to have them re-review their position to maximize your benefit. Typically there are very few deviations from an original estimate, however, we encourage you to consult with your insurance provider about your specific plan and its coverage. In the event of a discrepancy, we will hold you responsible for the complete treatment cost.
]What if we don’t hear back from your insurance company prior to your surgical appointment?
If you choose to proceed with your surgical appointment we will collect the full treatment fee from you, submit the claim to your insurance company, and have reimbursement come directly to you. If reimbursement comes to us, we will endorse the check to you. If you are scheduling your appointment within 30 days, and this situation is not amenable to you, please discuss alternatives to scheduling so that a last minute cancellation is avoided. Thank you.
